What happens if you require a visit at home?
You will call 111 and inform them that your registered GP is not within your home boundaries and you are registered under patient choice. They will organise for a local GP to carry out any necessary visits and follow ups and communicate that information to us, your registered practice.
If you are registered with us and move outside the practice boundary but want to stay with us, you will need to re-register with a ‘patient choice’ status and the home visiting rules will apply meaning 111 will support your needs in the event that you cannot attend your registered practice.
Not all patients will be appropriate for this type of registration or for remaining with the practice and a GP will need to approve the registration based on clinical risk and need. Patients with complex needs or multiple conditions requiring frequent monitoring would be advised to register within the boundaries of their home address. Each patient choice registration will first be approved by a GP Partner at Brackley Medical Centre.

Change of Contact or Personal Details
Please advise reception if you change your name, address or telephone number.
It would be a great help if you could also ensure that we have your postcode and your mobile number.
If you have changed your name on marriage, we will need to see your original marriage certificate and will need to take a copy.
